Q: If I (as the employee) terminate my employment 6 months prior to becoming eligible for Medicare and enroll myself and my spouse in COBRA, does my spouse lose COBRA eligibility at the time that I cease COBRA coverage and begin coverage under Medicare? She will need COBRA for 15 months after my retirement, while I will only need COBRA for 6 months after retirement.

A:  The covered employee that already has COBRA continuation and they become entitled to Medicare before the 18 months end, their spouse and dependents can stay on COBRA.   When this happens it is called a 2nd qualifying event.  The spouse or dependents will receive an additional 18 months of COBRA coverage for a total of 36 months.
