Q:I was laid off/terminated and my former employer has instructed me to sign a Request for continuation of medical and dental insurance.They will notify Blue Cross after that I am supposed to send my payment straight to Blue Cross. Is this correct and will Blue Cross know to only expect 35% of the premium from me?
