Q:  Just moved and lost forms for payment, and new rate. Who do I call to find out? Also is there refund for higher rate paid up to Stimulus plan?

A:  If you moved and lost the information to apply for Cobra, you need to contact your previous employer whom you had benefits through. They are the ones who supplied you with the information, and they may be able to resend that information to you.  As well as they are going to inform you if your Cobra coverage is subject to the reduction in premium, if you are interested in an alternative plan.  Which often times is more affordable than Cobra; it would cover all new illness and all new injury, feel free to click the following link:  Short Term Medical
