Q:  My previous employer did not make a payment on my behalf and the coverage was terminated on 6/30/07. How can I reinstate with retroactive benefits to 7/1/07?

A:  If the insurance company  has not received your payment then it is the the past employer or their 3rd party plan administrator whom you need to question. Since COBRA is a Federal law and if you think there is non compliance issues involved, you may want to contact the Department of Labor. They are a part of the government that enforces that law. You can contact them by calling 1-866-487-2365.
