Q:  Where do I find the Initial COBRA Form on this website? Also: I need to send the form to the employee and a separate mailing to his spouse and family correct? How do I mail it to show proof of mailing?

A:   To access COBRA forms you can:

1. 
Call your state EBSA (Employee Benefit Security Administration) office. You can find that state specific number if you visit www.dol.gov/ebsa  you can also call them at 1-866-444-EBSA

2. 
You can call the Department of Labor at 1-866-487-2365, or visit their web site at www.dol.gov.

 

You must send notification by first-class mail to both the employee and their spouse.  If the employee and the spouse live together you only need to mail one but addressed to both names.  You need to make copies of all COBRA correspondence that you mail out and you need to document each procedure;  what was sent, to whom it was sent, how the letter was address, the date it was sent and who did the mailing.
