Q:   I would like to know on which month and which day that my insurance will be discontinued.  Could you send me a copy of when I first received it and the end of my coverage?

 

A:   You need to contact your previous employer or their COBRA plan administrator.  Your previous employer should have sent you a “Notice of Termination or Cancellation of Health Benefits” letter.  This letter will state that your COBRA coverage has ended.  You will then receive a Certificate of Creditable Coverage for the time you were on COBRA continuation.
